Employees can create, display, change, delete and review the status of their health and dependent care claims through Employee Self Service (ESS) FSA/ITS Claims.

To receive reimbursement from your ITS account complete the appropriate form.  Please read the directions available on page 2 of the form.

Once completed, place the forms and supporting documents in a sealed envelope marked PERSONAL & CONFIDENTIAL, and send to Shari Coe, Auditor-Controller's Office.  Be sure to make photocopies of all forms for your personal records.
