Guiding Principles

MISSION STATEMENT:

"Working together to meet the health and human service needs of adults and seniors." 

PURPOSE: To provide leadership to all agencies, programs, and consumer groups serving adults and seniors; to provide leadership in the development of comprehensive community services to adults and seniors who utilize publicly funded health and human services. 

This will be accomplished by:

Improving communication, planning, coordination and cooperation among adult-serving agencies. Identifying critical issues for study, recommendation and action.

Identifying gaps and duplication in services.

Providing a forum for clarifying perceptions and expectations among agencies and between agencies and the community.

Establishing priorities for interagency projects and implementing collaborative programs, both public and private. Moving forward to create and implement an integrated system of care to better serve adult and aging community residents. 

The Adult Services Policy Council has decided upon a set of principles to guide the development of an integrated services system: 

Community-based services 

Development and implementation of regional health and human services. 

Access to services 

Comprehensive assessment and service coordination provided at point of entry. A continuum of services available in all communities. Countywide emergency response. 

Case Management 

Comprehensive case plans developed to meet the individual needs of consumers, in particular, those who use the services of several providers at the same time or in sequence. 

The Adult Services Policy Council developed and sponsored the Interagency Resource Team (IRT) that is available to provide multi-disciplinary case consultation.
