REQUEST FOR COMMUNITY BASED ORGANIZATION (CBO) AND PREVENTIVE HEALTH GRANT (PHG) GRANT APPLICATIONS

San Luis Obispo, California, December 18, 2007 - San Luis Obispo County has released a Request for Applications (RFA) for funding of community based organizations and preventive health grant programs in FY 2008-09.

Funds are intended to be used for local programs/projects that support and complement (not duplicate) the efforts of the County's health and human services departments in promoting the optimal health, stability, independence and well-being of County residents. Both public and private nonprofit organizations may apply. Programs and projects must be carried out in San Luis Obispo County.

Applications received will be reviewed by the CBO/PHG Review Committee, who will make recommendations to the County Administrator and the Board of Supervisors.

Applications are due in the County Administrative Office by FEBRUARY 4, 2008, 5:00 p.m. For a copy of the Request for Applications, or if you have questions, contact Nikki Schmidt, in the San Luis Obispo County Administrative Office at 781-5496. In addition, an informational meeting will be held on Thursday, January 3, 2008 at 3:00 a.m. in the Board of Supervisors Chambers, County Government Center, 1055 Monterey Street, San Luis Obispo. This meeting is not mandatory, but is intended to provide information about the application process, criteria for funding, answer questions, etc.
