County Self Assessment Questionnaire

The San Luis Obispo County Departments of Social Services and Probation seek your input for our County Self-Assessment.  The County Self-Assessment is part of a state mandated triennial review process that results in a System Improvement Plan.  The System Improvement Plan is our county’s commitment to both the state and the children and families we serve that we will take the steps necessary to address areas in need of improvement.

During the County Self-Assessment we are seeking input from our partner agencies and from the families of San Luis Obispo County, if you are interested in providing input, please answer the following questions by May 16th, 2008.

#5 From your perspective, how can the Juvenile Probation Department and/or Child Welfare Services contribute to increase the success rate of families who are involved in reunification services, thereby avoiding re-entry into care?
