Who are the Permanency Case Managers and what do they do?
The Independent Living Program has two Permanency Case Managers:

Permanency Case Managers work with foster youth that are:
- 10-18 years of age
- In Permanent Placement status
- Reside in San Luis Obispo County
- Do not have an identified plan for guardianship or adoption
Permanency Case Managers work intensively with a small number of cases at a time (typically 6-8).
Cases are prioritized by:
- Number of years in care. Youth in foster care the longest receive priority.
- Age. Youth nearest to Emancipation receive priority.
The primary duties of the Permanency Case Manager are to:
- Interview and complete assessments with the youth to identify significant individuals.
- Mine case files and CMS history to identify significant individuals.
- Complete a family tree for each youth.
- Contact the identified individuals to assess their level of commitment to the youth.
- Facilitate bi-monthly Permanency Case Staffing with the Social Worker, Social Worker Supervisor, and other relevant staff. Cases are discussed and action items are generated for follow-up. These take place at the Regional Case Staffings.
- Complete picture and profile to be included in the Adoptions Binder to be shown at foster parent trainings.
- Refer youth for other adoption outreach: Heart Gallery , etc.
- Complete a Guaranteed Preparation Packet for youth on their caseload upon Emancipation.
