The San Luis Obispo County Public Health Department maintains the registry of vital records for all births and deaths that occur in the County.  

The Vital Records Division

•         Records births and deaths
•         Provides birth and death certificates to family members who request them within two years of the birth or death
•         Registers amendments and corrections to birth and death certificates
•         Works with physicians, coroners and funeral directors to coordinate appropriate completion of death certificates
•     Information regarding the newly implemented Electronic Death Registry System (EDRS)

A fee is charged for a certified copy of birth or death certificate.

You can order a copy using your credit card by: faxing the completed form along with your credit card information to 805-788-2999; or calling 805 781-5514.

To request a copy of a birth or death certificate more than two years after the event, contact the County Clerk Recorder at 805-781-5080 or the State Registrar at 916-445-2684.
