Birth and Death Certificates

San Luis Obispo County maintains the registry of vital records for all births and deaths that occur in the County. Copies of birth or death certificates from the past two years can be obtained at the Public Health Department. Copies of births and death certificates that occurred more than 2 years ago can be obtained from the San Luis Obispo County Clerk-Recorder, located at 1055 Monterey St. in downtown San Luis Obispo. 

The Vital Records Division

  •         Records births and deaths
  •         Provides birth and death certificates to family members who request them within two years of the birth or death
  •         Registers amendments and corrections to birth and death certificates
  •         Works with physicians, coroners and funeral directors to coordinate appropriate completion of death certificates
  •        Information regarding the newly implemented Electronic Death Registry System (EDRS)
