The San Luis Obispo County Public Health Department maintains the registry of vital records for all births and deaths that occur in the County. Printed copies of birth or death certificates from the past two years can be obtained at the Public Health Department. Copies of births and death certificates that occurred more than 2 years ago can be obtained from the County Clerk Recorder's Office, located at 1055 Monterey St. in downtown San Luis Obispo. For copies from the Public Health Department, you can order a copy using your credit card by: faxing the completed form along with your credit card information to 805-788-2999; or calling 805 781-5514.

To request a copy of a birth or death certificate more than two years after the event, contact the County Clerk Recorder at 805-781-5080 or the State Registrar at 916-445-2684.
