After you complete and provide the required forms listed below, and they have been reviewed by Environmental Health Services, a facility record will be created and you will be able to apply for access to our Public Portal at that time. An Environmental Health Specialist will review the forms submitted and conduct a facility inspection to verify the accuracy of the information provided and ensure that your business is in compliance with all applicable laws and regulations.

Businesses handling hazardous materials at or above the reportable quantities of 55 gallons of a liquid, 500 pounds of a solid, 200 cubic feet of a compressed gas, radioactive materials in quantities for which an emergency plan is required, extremely hazardous materials in quantities equal to or greater than the applicable federal threshold planning quantities, or that generate hazardous waste must complete and provide the
Group A Forms to Environmental Health Services.
