New Web Portal User Instructions and Forms

If your laboratory, facility, physician's office, clinic or water testing location submits specimens for laboratory testing to the County of San Luis Obispo Public Health Laboratory, you can now view results online!

1.	Supervisor must download, save and then fill out the Web Portal User Registration Spreadsheet (when prompted, select "SAVE" to download, save and print the spreadsheet) for each potential user. Once complete, email it to the County of San Luis Obispo Public Health Laboratory, jbeebe@co.slo.ca.us, instructions are on the form.
2.	Each user listed on the Web Portal User Spreadsheet must sign and return the Web Portal User Agreement to the County of SLO Public Health Laboratory by U.S. mail or courier (Dr. JL Beebe, SLO PH Laboratory, 2191 Johnson Ave., San Luis Obispo, CA 93401).
