All clients have a right to confidentiality. The Mental Health employees of the San Luis Obispo County Health Agency, Mental Health affiliates, and Mental Health contract providers know that health information about you and your mental health is personal. They promise to protect your health information. A record of care and services you receive at Mental Health Services is made and stored at Mental Health offices. This record is needed to give you care and to meet the laws of the state of California. This notice applies to all records of your care kept by Mental Health Services, whether made by Mental Health staff or one of our contract providers.
Mental Health Services records and stores client information in many places, both on paper and in computers. Mental Health care workers and doctors share this information with one another in order to care for your mental wellness.
This notice tells you about the ways that Mental Health Services may use and share health information about you. It also tells you about your rights and certain rules Mental Health Services has about how information is used and shared.
The law requires Mental Health Services to:
· keep a record of the care it provides you
· make sure that health information that could be used to identify you is kept private
· give you a notice of privacy practices; and follow the notice that is in effect at this time
