The Homeless Outreach Program is designed to serve mentally ill homeless adult.  This program assist clients in accessing resources for mental health services, primary health treatment, and social services in support of sustained community living. 

The Homeless Outreach Program is a collaborative program between the County of San Luis Obispo Behavioral Health Services and Transitions-Mental Health Association.
