Services specific to transitional age youth

Transitional age youth service is a new program being offered in the spring of 2006.  This is an enrollment based program for 20 transitional age youths ages 16 to 21.  The service offers intensive integrated Wrap-like mental health services with access to care 24 hours a day 7 days a week.  The service focuses on mental health, housing, employment services, substance abuse treatment and independent living skills. 

The target population are youths with one or more of the following characteristics:  chronic history of 5150, hospitalizations, ER visits, law enforcement involvement; dual diagnoses; foster youth with multiple placements, or aging out/have aged out; recently diagnosed (could be identified vie juvenile justice system).
