The San Luis Obispo County Public Health Department provides programs
to promote health and prevent disease for infants, children and teens in the community.
Child Health and Disability Prevention Program (CHDP)

- Provides free health and dental check-ups to help children stay healthy, find health problems early, and answer health questions.
- Offers examinations that focus on disease prevention for infants, children and teens; also available for school entry, sports/camp physicals and foster care placements.
- Includes a complete health history, head-to-toe physical exam, growth and development check, nutrition information, vision and hearing tests, immunizations, tuberculosis (TB) tests, lab tests, and health education.
- Provides dental and medical referrals, transportation information, and assistance in making appointments.

CHDP Gateway to Health Coverage
If your child is eligible, he or she can get health care services paid for by Medi-Cal until the end of the month following the application.
CHDP Gateway allows providers to pre-enroll children into temporary, full-scope, no cost Medi-Cal based on apparent eligibility. Services include dental, vision, prescriptions, doctor visits, and more.* You may apply for
Medi-Cal,
Healthy Families, or
Healthy Kids to continue the health coverage. Until Medi-Cal, Healthy Families, Healthy Kids or other insurance is in place, CHDP Gateway will help you get the health care your child needs when sick, and the care he or she needs to stay healthy.
*Subject to provider participation.

- Not on Medi-Cal, and
- Have low to moderate income, and
- Are 0 to 19 years old.
