Communicable Disease Control and Prevention

Welcome to the Communicable Disease (CD) Control and Prevention Program in the Public Health Department of San Luis Obispo County. The CD program works in partnership with healthcare providers and the community to protect the public from the spread of contagious diseases that could result in illness, disability or death. The CD Program also provides consultations and information on contagious diseases, offers referrals for treatment of individuals exposed to certain communicable diseases, and conducts investigations on highly transmissible conditions.

The key functions of the Communicable Disease Control and Prevention Program include:

- Communicable Disease Surveillance and Control:

  • Conduct surveillance for over 80 communicable diseases
  • Investigate and control outbreaks of disease within the Community
  • Collaborate with community partners on special projects, studies and investigations
  • Collate information and produce reports on disease incidence in San Luis Obispo County

-To report a Communicable disease, please use a Confidential Morbidity Report (CMR) ( PDF 1064 KB) This form is for all diseases OTHER than Tuberculosis and a lapse of conciousness and/or condition reportable to the DMV. For Cocci (AKA Valley Fever, use this form)

-If you are a healthcare provider or a field responder and have been exposed to a contagious disease, please complete a Contagious Disease Exposure Report Form and fax to the County Health Officer

- Education and Prevention:

  • Provide education about Communicable Diseases
  • Consulting services to Medical Providers on issues of infectious and communicable diseases
  • HIV and Hepatitis and counseling to high risk populations and community agencies
  • Post exposure recommendations and referrals for communicable disease
