We respond to several types of public health complaints, including concerns about failing septic systems; illegal dumping or trash buildup; waste tires; hazardous material violations and vector control.

SLO County CUPA Business Plan Program
The CUPA.zip file contains the forms, instructions, lists, and examples for the SLO County CUPA Business Plan Program in Microsoft Word 97. Click this file CUPA File Package and you will be prompted to either "Save this program to disk" (you can run it later offline to extract the files) or "Run this program from it's current location" (it will automatically run and unzip the files into the following directories on your hard drive):

Extracting the forms into these default directories will save you some time sorting out which of the extracted files goes into which category. After extracting the files, review the reporting requirements and accompanying flowchart (
PDF 78 kb) to determine whether you qualify for the program, and then review the instructions for each form in the folder C:\temp\Instructions. Examples of filled out forms for each form type, e.g., frm-i.doc, the chemical inventory form, are provided in the Examples folder. You will also find pre-filled out forms for some commonly used chemicals in the Examples folder. To save time filling out the entire form, you can use the pre-filled out forms and just put in information that is pertinent to your business, for example, the chemical’s quantity, container type, and location information.
