Instructions for manual submission of CUPA forms

Step 1: Review the HMBP Program Eligibility Flowchart to determine whether you qualify for the program

Step 2: Review the instructions for each form (see table below)

Step 3: Download the appropriate form or forms

Step 4: Fill in the required information and submit as instructed

GROUP A FORMS: Businesses handling hazardous materials at or above the reportable quantities of 55 gallons of a liquid, 500 pounds of a solid, 200 cubic feet of a compressed gas, radioactive materials in quantities for which an emergency plan is required, extremely hazardous materials in quantities equal to or greater than the applicable federal threshold planning quantities, or that generate hazardous waste must complete the forms in the box below
