Thank you for your interest in the CHDP Program.  Every local health department in the State of California has a Child Health and Disability Prevention (CHDP) Program. The CHDP Program is responsible for the implementation of standards and guidelines for the provision of preventive health services to eligible children in California. 


Health Assessments

Children are eligible for health assessments according to the American Academy of Pediatrics' (AAP's) Recommendations for Preventive Pediatric Health Care.

Components of the health assessment include:


Care Coordination

The local CHDP Program will assist families in:
  • Finding affordable health insurance
  • Locating information on available health services
  • Locating Medi-Cal Physicians and Specialists
  • Public Health Nurse availability
  • Health Education Outreach
The CHDP Program is responsible for meeting the federal Early and Periodic, Screening, Diagnosis and Treatment (EPSDT) requirements for children.  Providers may call the local CHDP Program and request assistance for their patients.

Provider Enrollment

Our goal is to ensure that eligible children and youth have access to ongoing health care .  In San Luis Obispo County, there are 17 public and private medial offices and clinics, including pediatricians, family practice physicians, physician assistants and nurse practitioners, approved to provide CHDP services.  If you are interested in becoming a CHDP Provider, please click here.  Or you may contact your San Luis Obispo CHDP Program staff.


Referrals

Children identified with suspected problems after a health assessment and screening are referred for necessary diagnosis and treatment.  Many problems can be prevented, corrected, or reduced in severity by prompt diagnosis and treatment.
