Below are forms you will need. If you live in Los Osos or Santa Margarita, you'll need the the Los Osos Moratorium form or the Santa Margarita Fire Plan listed below, too.

Start here:
Construction Permit Submittal Checklist
The Checklist outlines everything you'll need to submit with your application.

Then get:
Construction Permit Application Package
The Construction Permit Application Package includes Consent of Landowner form, the Construction Permit Application, a Division of Occupational Safety & Health (DOSH) clearance form, an Information Disclosure form, and a list of San Luis Obispo County Identified Hazardous Waste Sites.

Building in Los Osos?

In addition to the forms above, you'll need:
Los Osos Moratorium Statement of Compliance
This form is required for all construction projects in Los Osos.

Building a home in Santa Margarita?

In addition to the forms above, you'll need:
Santa Margarita Residential Project Fire Plan Request
This form is required for all construction projects in Santa Margarita. 

Have a smaller project that isn't available through E-Permits?

For smaller projects that are not currently available through E-Permits, you'll need:

Request for Allocation/Allotment Application  package.  This package includes the Allocation/Allotment Application form and the Consent of Landowner form.
