Public Health Clinics
To make an appointment at a Public Health Clinic, please call (805) 781-5500 or the individual clinic at the numbers listed below. Our hours are 8 am - 5 pm.
COVID-19 vaccine appointments now have an office fee of $10.00 each that can be covered by insurance, including Medi-Cal. Public Health clinics serve all patients, regardless of ability to pay.

Come Prepared
- Complete, print, and bring your vaccine screening form / formulario de examen para vacunas (forms also available on-site).
- Bring documentation showing your age (driver’s license, state ID, school ID, doctor’s note, passport, etc.)
- Please wear short sleeves or other clothing that makes it easy to access your upper arm.
- Be prepared to wait 15-30 minutes for observation after you receive your vaccine. The entire process may take up to one hour, including registration, screening, vaccination, and the observation period.
For parents:
- Children (under the age of 18) must have an accompanying parent or guardian with them, as well as documentation of their age, such as a passport, birth certificate, medical record, school ID, etc.
- Parents: please review and sign the forms available in English and Spanish before your child’s vaccine appointment (forms also available on-site).

COVID-19 Boosters
If you have not yet gotten a bivalent COVID-19 vaccine (bivalent meaning it protects against both the original COVID-19 strain and the current Omicron strains – this is the updated “Omicron booster” rolled out in fall of 2022) then you are due for an updated COVID-19 vaccine. If you have already gotten a bivalent booster, you are eligible for another one if you are age 65+ or have certain health conditions.
Who is currently eligible to receive an updated (bivalent) COVID-19 vaccine:
- Anyone age 6 and older who has not yet been vaccinated against COVID-19 may now receive a single dose of the updated (bivalent) vaccine rather than a multi-dose primary series with the original formula.
- Everyone ages 6 years and older who received their original COVID-19 vaccine series but has not yet received an updated (bivalent) booster may get a booster now.
- Adults age 65 and older who have already received an updated (bivalent) booster may get an additional booster, 4 months after their first.
- Those with certain immune compromising conditions who have already received an updated (bivalent) booster may get additional boosters. This allows more flexibility for healthcare providers to administer additional doses to immunocompromised patients as needed.
- Individuals ages 6 years and older who have already received an updated mRNA vaccine do not need to take any action currently, unless they are 65 years or older or immunocompromised.
- For young children (6 months–4 years), multiple doses continue to be recommended and vary by age, vaccine, and which vaccines were previously received.

Lost your vaccine card?
To get a new vaccine card, walk in to any one of our vaccine-providing Public Health Clinics, bring your photo I.D., and the team there can re-write you a new one.
You can also access a digital copy if you got vaccinated in CA. If you want to share your proof of vaccination, you can either use the digital version or the card you were given at the time of your vaccination.
