Child Health and Disability Prevention Program: Vision Screening Training
This is a certifying course that will provide participants with the necessary knowledge to conduct vision screenings of children ages three through 21 in a CHDP provider’s office.
Participants who complete the training must complete the post-test and the evaluation form in order to receive a Certificate of Completion. Recertification is recommended every four years. Please feel free to use all the resources on this page and save or print them for your reference.
This training is intended for CHDP providers in San Luis Obispo County. (CHDP providers in Santa Barbara County may use this training as appropriate but may be asked to present additional competency to their local CHDP program.) Providers in other counties should contact your local CHDP program for local training opportunities and resources.

Please follow these steps to complete the training.
Step 1: Complete the pre-test.
Download and complete the vision screening pre-test.
Step 2: View and complete the Vision Screening Presentation Part A.
Download the vision screening presentation and review the material on each slide.
Step 3: Watch the training videos.
Step 4: View and complete the Vision Screening Presentation Part B.

Step 5: Review the state vision screening guidelines.
Download and review documentation resources and requirements.
Step 6: Complete the post-test.
Download and complete the vision screening post-test.
Step 7: Complete the training course evaluation.
Download and complete the training evaluation.
Step 8: Submit the post-test and evaluation.
Submit the completed post-test and evaluation by email or fax:

After we receive these documents, we will send you a certificate of completion.
