If food items to be sold or distributed from the temporary food facility will be prepared in advance of an event at an alternate food facility location, i.e., not within the temporary food facility, approval of that alternate location is required in order to approve the permit to operate. Additional forms are required along with the health permit application.

Submit the following forms along with the permit application (also included in the forms packets above):

Pay the Health Permit Fee

The permit fee is due with the application. Please refer to the Environmental Health Permit Fee Schedule for more information.

Await Approval From Inspector

It is unlawful for a Temporary Food Facility to operate at an event without prior approval from Environmental Health Services.

The health inspector will review your completed application and sign off when approved. The inspector reviewing the application will contact you with any additional information that may be required, revisions needed, or when your application is approved. For Multiple-Event applicants, a paper Health Permit to Operate will be mailed within 2-4 weeks after receiving inspector approval.
