VOYA

Who can use this service?

These voluntary plans are only available to permanent County employees working a minimum of twenty (20) hours per week.

What is the process?

Step 1: Check Eligibility

New employees can enroll in voluntary plans within the first 31 days of employment with a guaranteed issue for short term disability, long term disability and up to $150,000 for employee life insurance, without submitting an Evidence of Insurability (EOI) form. After that initial 31 days, employees cannot enroll in Voluntary Benefits until Open Enrollment of each year and EOI requirements will apply. It is the employees responsibility to complete an EOI form which requires additional information about your health and the health of your dependents you want to cover. You must submit the  EOI form to Voya for them to determine your eligibility. No deductions will be taken from your paycheck until the EOI has been approved by Voya.

Voya plans sponsored by the County are group plans and Voya does not maintain individual eligibility or enrollment information for these plans. All plan documents are provided below.

Life Insurance pays your beneficiary a lump sum if you die. AD&D provides another layer of benefits to either you or your beneficiary if you suffer from loss of a limb, speech, sight, or hearing, or if you die in an accident. Coverage is available for you, your spouse, & children. The employee must be approved for coverage to add dependents. 

The benefit amount reduces to 65% at age 65, to 50% at age 70 and to 30% at age 75.

Supplemental Life and AD&D

  • This benefit is available to all eligible employees through Voya Financial for the 2017 plan year. Employees can apply for supplemental coverage in increments of $10,000, the minimum coverage amount is $20,000 with a maximum of $500,000.
  • New Hires: Employees can elect up to $150,000 without providing evidence of insurability (EOI) if they apply within 31 days of first becoming eligible. An EOI form will be required for all other enrollees.

  $20,000 OR 50% of employee amount

  $10,000, not to exceed 100% of employee amount

 New hires who elect Voluntary Life within their first 31 days of employment receive a guaranteed issue amount without having to submit Evidence of Insurability form to Voya

  • Beneficiary Reminder: Make sure that you have named a beneficiary for your life insurance benefit. It's important to know that many states require that a spouse be named as the beneficiary, unless they sign a waiver.
  • Taxes: Due to IRS regulations, a life insurance benefit of $50,000 or more is considered a taxable benefit.

Voya Short-Term Disability Insurance

Group Policy Number: 31640-7

  • This benefit can replace a portion of your income during the initial weeks of a disabling illness or accident. The weekly benefit amount is 60% of your covered weekly earnings. The maximum weekly benefit is $1,325. 
  • This plan lasts for 12 weeks and has a 7 day wait period before benefits are paid.
  • New Hires: Employees can enroll without providing an evidence of insurability (EOI) form if they apply within 31 days of first becoming eligible. An EOI form will be required for all other enrollees.

Voya Long-Term Disability Insurance

  • This benefit can replace a portion of your income if you can't work because an injury or illness prevents you from performing any of your functions over a long time.  It's important to know that benefits are reduced by income from other benefits you might receive while disabled, like workers' compensation and Social Security. 
  • This plan pays 60% of monthly covered earnings up to a $10,000 monthly maximum and begins after 360 days of disability. 
  • Only certain Bargaining Units not already covered by an LTD Policy can participate.

Step 3: How to Submit a Claim

Voya Short Term and Long Term Disability Claims

  1. Start the process 2 – 4 weeks in advance of your disability date. Internal processing takes about 1 - 2 weeks and cannot be completed until date of disability
  2. Provide a copy of the Disability Income Phsyicians Statement to your doctor.
  3. Download and complete the employee disability form.
  4. Submit the employee form to Voya per form instructions or Online at Voya’s Claim center. Since this is a group plan, Voya is unable to answer questions related to individual claims until an employee claim form is submitted.
  5. Email [email protected] and [email protected] or Interoffice mail to HR Benefits with a request to complete the employer disability form. Include a copy of your employee forms or the following information:
    • Employee Name
    • Date of Birth
    • Social Security Number
    • Whether Applying for Short or Long Term Disability
    • Last Day at Work
    • Date of Disability
    • Whether or not the disability is work related
    • Copies of Approved Leave of Absence (LOA) Forms
  6. The County will submit the completed form to Voya for processing. The County does not receive any information regarding the status of your claim.
  7. Once a claim has been submitted, you are able to contact Voya directly on questions related to your claim or payment of your claim.

Is there a charge for this service?

Rates for Short and Long Term Disability will change with salary and age throughout the life of your plan. Please see BenXcel.net during OE for your personalized premium.

Monthly premiums vary depending on the type of insurance and amount of coverage selected. Monthly premium payments are deducted in two (2) installments from the first two (2) pay periods of each month. They appear as after tax deductions on the employee's remuneration statement.
