Evacuation Assistance List and Registration
What is this service?
The San Luis Obispo County Office of Emergency Services maintains a list for independent living individuals who would require special evacuation assistance following an emergency at Diablo Canyon Power Plant.
Who can use this service?
This list is for independent living residents within the Emergency Planning Zone (EPZ) who would be unable to evacuate themselves due to a disability or medical need. However, residents on this list that are able to leave the area through other means, such as family, friends or neighbors, are encouraged to do so. Evacuation assistance out of the affected area would be provided as resources became available.
Is there a charge for this service?
This service is provided free of charge.
What is the process?
Step 1: Obtain Evacuation Assistance Request Card
Download and print the Evacuation Assistance Card.
FYI - Residents living in the Emergency Planning Zone will receive an Evacuation Assistance Card each year in the PG&E/SLO County Emergency Planning Calendar mailed out each fall.
Step 2: Complete the Evacuation Assistance Request Card
Complete all fields on the Evacuation Assistance Request Card.
Step 3: Submit Evacuation Assistance Request Card
Submit the card to:

Step 4: Receive Confirmation
Once your card has been received and processed, you will receive confirmation that you are on the Evacuation Assistance List.
Step 5: Maintain Presence on List
To maintain your status on the list from year to year, fill out an Evacuation Assistance card each year.
When and where is this service offered?
This service would be offered during an emergency at Diablo Canyon Power Plant that necessitated an evacuation. Those evacuated will be transported to a Reception Center for registration, family reunification, or transfer to a evacuation shelter as needed.

Where can I get a copy of the calendar that has the card in it?
The calendar is mailed out to all residences in the Emergency Planning Zone each fall. The calendar is also available for pick-up at various county buildings throughout the county. The calendar is also available on our website. Contact our office at (805) 781-5011 if you have any questions.
