Application form for a Voluntary Merger (PLN-1120). This form must be accompanied by the other forms listed on the 'Voluntary Merger Application Forms'.
General Application and Contact Information form (PLN-3000). This form is used for every type of Land Use and Land Division application.
Information Disclosure Form (PLN-1006)
Land Use Consent of Property Owner (PLN-1012). This form is required when the applicant is not also the owner of the property.
