Voluntary Merger - Attachments Required for an Online Submittal
This checklist lists below identifies all attachments needed to apply online for a voluntary merger. When reviewing the checklist, you will need:
NOTE: *** Please ensure all documents reflect the voluntary merger number. Obtain a voluntary merger number here, if you have not already.
- Voluntary Merger Application - completed and signed
- In the, Land Division Application - complete and sign the following pages:
- (PLN-3000) ‘General Application Contact Information’
- (PLN-1006) ‘Information Disclosure'
- (PLN-1122) ‘Hazardous Waste and Substances Statement Disclosure’
- Title Report or Lot Book Guarantee dated - within the last six months for each parcel
- Consent of Property Owner (if the applicant does not own the property)
