Participation in foster care after the age of 18 is called Extended Foster Care. Some youth, who were previously in foster care as wards of the Delinquency Court, are eligible to voluntarily return to foster care after the age of 18.
If you are an eligible youth who would like to request to re-enter Extended Foster Care, complete the attached JV-466 form. If you wish to keep information regarding your re-entry request confidential, complete the JV-468 form. You can submit these forms through the Probation Department or directly to the Juvenile Delinquency Court.
If you are unsure if you are eligible or would like additional information on re-entering Extended Foster Care, please contact Juvenile Services.
