ATP provides a home-like setting in San Luis Obispo, CA, where adults with mental illness are supported in their efforts to acquire and apply interpersonal and independent-living skills. ATP is a transitional program with a maximum stay of 12 months; it is licensed to house 12 people and is staffed 24-hours.

FAQs

Yes, community housing is available through Transitions Mental Health Association. Individuals must be referred by their County Mental Health provider to be considered for community housing.

For questions regarding the conservatorship process, please contact the Public Guardian’s Office:  (805) 781-5845. The individual must be assessed by designated professionals to determine that they meet the criteria for grave disability due to severe mental illness.

If your family member is currently receiving County Mental Health Services, you may talk with their provider about the possibility of placement in one of the Adult Mental Health Residential Programs based on availability and ability to pay.

To be housed in any of the Secure Adult Mental Health Residential Programs, the individual must be first be conserved by the County of San Luis Obispo Public Guardian’s Office.
