Voluntary Merger - Attachments Required for an Online Submittal

This checklist lists below identifies all attachments needed to apply online for a voluntary merger. When reviewing the checklist, you will need:

  • Voluntary Merger Application - completed and signed
  • In the, Land Division Application Package - complete and sign the following pages: 
    • (PLN-3000) General Application Contact Information
    • (PLN-1006) Information Disclosure'
    • (PLN-1122) Hazardous Waste and Substances Statement Disclosure
  • Title Report or Lot Book Guarantee dated - within the last six months for each parcel
  • Consent of Property Owner (if the applicant does not own the property)
