Construction Type, Building Occupancy, Stories, Bedrooms, Bathrooms, Dwelling Units, Recycling Plan (choose beteween Detailed, Integrated Waste Management Authority IWMA or N/A), Sewer Lateral Connection (Existing, None, New), Choose either Well or Community Water, Total Area of Disturbance (Acres).

Note:  If you choose "None" for Sewer, then check "Septic"
