How to Request Records
Vaccine Records
You can get a Digital Vaccine Record online at the California Department of Public Health website, including vaccines you have received from our Public Health clinics.
Other Public Health Records
If you need health information for other services from Public Health Clinics — such as reproductive health care, nurse home visiting, physical or occupational therapy, cancer screening, STD testing, TB testing, or other services — please contact us by email or call 805-788-2138.

Behavioral Health Information
If you are looking for health information from Mental Health or Drug & Alcohol treatment, please reach out to the Department of Behavioral Health by calling 805-781-4724 or contact the team by email. If you are currently receiving Behavioral Health services, you may request Behavioral Health Information from your provider.
Jail Medical Information
You can request records of medical care you received while at the County jail.
- For care received before February, 2019, please contact us by email or call 805-788-2138.
- For care received during or after February, 2019, please complete this form and send by fax to 805-781-5342 or mail it to Wellpath Medical Records, 1585 Kansas Avenue, San Luis Obispo, CA 93405.
General Hospital Records
If you are seeking records from care you received at the County General Hospital that closed in 2003, you may reach out to your primary care provider from that time.

Records Potentially Related to Reproductive Health Care
If an agency requests health information that may be related to reproductive health care, it must complete an attestation form before accessing private health information. This helps ensure the request complies with HIPAA privacy requirements. This process is required for anyone needing health information who is not the patient themselves or the patient’s legal guardian. The attestation must be filled out completely and faxed to (805) 781-5543 to request health information.
