Continuity of Care

Any member who has any qualifying medical condition with a non-contracted provider will need to complete a Continuity of Care form. (See form below).

Qualifying medical conditions include:

  • Serious chronic conditions
  • Pregnancy
  • Surgery or treatment already pre-authorized at the start of next year
  • Terminal illness

Completed forms should be sent to Blue Shield in December. Do not send forms earlier!
