What is this service?
San Luis Obispo County is home to a vast and diverse range of dedicated individuals and organizations working to create a healthy and vibrant future for our community. More than 95 community partners—including service providers, advocates and subject matter experts—came together to identify health priorities and plans for action over the next five years. Check out the community health improvement plan to learn more about each priority area and the goals, objectives and activities underway.

The community health improvement plan for San Luis Obispo County will guide our community’s strategic directions and priorities related to health over the coming five years. It describes how the Public Health Department and our partners will work together to improve our community’s health, sets forth what we will strive to achieve and provides a road map for how we plan to achieve it. It identifies priority health issues, establishes measurable objectives for improvement and coordinates resources to achieve those objectives.
