To report an outbreak of COVID-19 (i.e., 3+ cases in a cohort), please email [email protected] with the following details:

How many staff cases
How many student cases
Name of cohort (e.g., Ms. Smith’s 2nd grade class, JV boys basketball team, etc.)
Which school
Date of first case’s symptom onset (or date of first positive test if no symptoms)

As workplaces, schools and child care settings are subject to COVID-19 workplace outbreak reporting requirements as stipulated in Cal/OSHA COVID-19 Non-Emergency Regulations
